Occasional problems shifting into second gear

I have occasional problems shifting to second gear from first gear.

Not many times. I have to release clutch and try again, always works second time - but physically not possible if I don't release clutch.

Since I owned my car, I've always had the same with reverse - say 1 in 100 times.

What should I do?
What do I need to have looked at?

Is it a simple thing like adding oil to the gears or something?

1st to 2nd - is that when the car is first started or when the car has been driven for a while...or both? Any grinding of gears?

Is there any sign of an oil leak in the gear/clutch housing vicinity.

Reverse on most cars is an awkward select from time to time. Not often

I think some background info will help

How old is the car.
Has the Fluid been changed in the hydraulics
How many miles covered and in what type - IE - motorway , towing , around town.
Service history

For me , its an indication of clutch wear which doesnt appear immediately obvious as the system constantly adjusts itself.

@calnorth: no oil leaking.

@Buffy:
- Car is 66 plate, so 5 years old.
- fluid changed in hydraulics? Not sure what you mean. Please elaborate.
- mileage is 19,000 - 99.5% around town.
- service history, I got 1 year ago. Gave it a full service when I got. Not sure abut what happened before. There was little paperwork. When I got the car, I think it had 11,000 miles, can't remember, it was low though.

Problem happens still on the odd occasion still.

Hydraulics (fluid) is the method by which the clutch pedal tranfers foot force to the clutch. Used to be steel cable.

1st to 2nd is unique, meaning no other gear is selected. That requires specific gears on two shafts to be at the correct speed to mesh silently....synchronised. Where you repeat operating the clutch will change the speed of the gears and in your case, and at the time allows the gears to engage silently. So your initial selection didn't place gears at the right speed to mesh and caused grating.

A faulty clutch could cause the problem but I'd expect to see it with other gears.... 3, 4, 5, 6, R

A faulty synchronisation ring for 2nd gear is possibly the problem...or that area anyway.
